Media Consent and Release Form

I hereby grant permission to Green Lake Conference Center and its representatives to take photographs and/or video recordings of me and to use these images and video in promotional materials, including but not limited to websites, social media, printed materials, and other media, without compensation or further approval. I understand that these materials may be used in a variety of settings and will be accessible to the public. However, I understand that upon request, my image will be removed from future use whenever reasonably possible determined by the GLCC marketing team.

I acknowledge that all such images and recordings are the property of Green Lake Conference Center, and I waive any right to inspect or approve the finished product wherein my likeness appears.

I release and hold harmless Green Lake Conference Center, its employees, volunteers, contractors, and agents from any claims, damages, or liability resulting from or related to the use of the images or recordings.
